Consultation

The first appointment may be for a diagnostic examination and to discuss treatment plan options. The purpose of the consultation is to allow time for review of important details, such as patient health history and current medications being taken, and to determine a treatment plan that is most appropriate for the patient. Often times, the consultation can take place at the time of the procedure. In certain circumstances, such as with implant treatment cases and patients who have extensive medical history, a consultation is needed prior to the procedure appointment.

Please assist us by providing the following information at the time of your consultation:




IMPORTANT: All patients under the age of 18 years of age must be accompanied by a parent or guardian at the consultation visit.

Please alert the office if you have a medical condition that may be of concern prior to surgery (i.e. diabetes, high blood pressure, artificial heart valves and joints, etc.) or if you are on any medication (i.e. heart medications, bisphosphonates, aspirin, blood thinners, etc.).




X-Rays
If your dentist or physician has taken x-rays, you may request that they forward them to our office, however if there is not enough time, please pick them up and bring them to our office. If additional films are necessary, they may be taken at our facility.
